No you may not use Methadone and I don't understand why you would want to because of Methadone's long half life. You may tell a pt how to taper the Methadone already prescribed. According to federal law, one may only prescribe Suboxone or Subutex for opiate addiction and then only with a DEA waver which we all have. Methadone may only be used by a certified clinic. Pt's opiates may be tapered, but not prescribed. In my practice, I never do that but rather expain to the pt that they need to be in WDRL before taking the first dose of bupe so I just have them stop their non-long acting opiate about 24 hours in advance of their appointment.
