We have used low dose Suboxone (4 mg) in a ambulatory withdrawal treatment protocol in all comers incl alcohol and benzo dependent/abusers without adverse effects noted. We do exclude benzo positive patients from longer term, higher dose Suboxone therapy.; Balto, Md.

I also have a patient on xanax for panic disorder and wonder if the use of this precludes using buprenorphone. Is there a dose limit?

From Feb to Sept '03 we did a few inpatient Suboxone-assisted opioid detoxes and inductions to maintenance while concurrently using Librium or Serax for alcohol or benzo detox. Now we're doing mostly outpatient and have one patient with severe panic disorder who is stable on both Klonopin and Suboxone. As far as I've heard, the buprenorphine deaths associated with alcohol or BZD's have been parenteral bupe. This is why I don't use any Subutex. ~Bill Grass
