I definitely would start with 16mg per day to start. With his history would induce in office cautiously. Probably would check daily until feeling good. Then after 1-2 weeks decrease to establish the maintainance dose.

I'm in Ohio and I look up on OARRS every new patient prior to scheduling their first appointment. Then I can discuss any issues before the first appointment. Also, it is reviewed day before each visit.

Thanks, I didn't not tell the caller she was a patient. I just thanked her. I have discussed and of course she has denied. Thanks for the input.

I have a patient that was on 11/2 strips Suboxone daily. She had a full mouth extraction. I increased to 1/2 q 6h. She also used 800mg Motrin q 6h X 2 days then stopped the Motrin. On day 10 she went back to 1 1/2 strips a day and did fine.

I usually start on 16mg then try to decrease to 12 after 4 weeks. Will then try to go down after that to fund maintainable dose. I have better retention with this than a low dose and going up
