Thanks for these responses. No he's not on vivitrol.

Very low-dose of ntx. Gets a little paranoid on a higher dose. He's on a high dose of Neurontin

Since he has so many side effects to ntx, might not be a loss to stop

Does anyone know the average opioid content of a Purim "mun" (poppy seed) hamentashen (pastry)? Or where to get reliable info?

Thanks to everyone who posted here. My apologies for being away from my PC and not responding sooner. RBR, good idea about the naltrexone, but we tried this already and pt refused it, claiming that it caused "jumpiness." At this point, the issue is moot. The facility "found" their MD and agreed to t...

rbr1, thanks for your response. The pt was started on naltrexone by the IOP, and was supposed to attend 3x/wk. Judging by the message left for me by the btr, the family was not contacted during the pt's short IOP stay, even though that was part of the protocol. My guess is that the little bag of rit...

I spoke w/ Aetna, and they said that they consider bup detox to be an ambulatory detox and would be willing to contract on a case by case basis w/ OP providers who do not already have a detox contract w/ them (which probably means anyone who is not affiliated w/ an IOP or PHP). It was not clear to m...

Peterorrin, Disclaimer: I have not yet prescribed any suboxone, having just received my official waiver last wk. Having said that...I personally would NOT Treat ANY psychiatric or CD pt w/o identifying data, and I would insist on having an emergency contact name/number as well. I would also wonder i...

This info will not help you w/ the practicalities of detoxing this pt, but there is a fascinating University of Chicago Press book entitled "Bread of Dreams," by an Italian author whose name I forgot. This book details the historical use of opium-based bread in Italy, dating back to the Roman Empire...

vidalia--thanks for the info. sorry to hear of these difficulties, but they make a lot of sense. I find it difficult to prescribe an SSRI w/o therapy, and am hardly surprised that opiate-dependent pts do not do well w/ meds alone. You raise some really important questions. rbr1-"floral opium" is opi...
