Re: duration of Bup withdrawal varies from fast to never...average to zero is several months by mutual agreement. One of my patients is a parochial school teacher with a young family. He has been on suboxone 1 mgm for a year. He does not go to more than 2 self help meetings/month. Another patient is...

"Abstinence only" inpatient programs ignore brain science, while adhering to an out-of-date economic plan. These rehabs are islands of care when multi-year recovery of care linkages are called for. RE: MAT. patients discharged from rehabs are at increased risk of overdose death. Every patient readmi...

I practice in a building at a curve of a busy 2 lane State highway in a suburban-exurban community. If someone is intoxicated clinically & by breathalyzer, we take their keys, and ask a friend/relative to pick them up and drive them home. I have actually driven a couple home myself. CT law freeing NP/APRNs from direct supervision causes concern here because practicing high quality addiction medicine is difficult, complicated, and it takes a long time to 'get good' at it. Psychiatrists and primary care docs should check with their insurance carriers to find out if probuphine implantation will increase rates. I will not be a pharmaceutical mfg's bank. Im not going to pay for a drug, then wait for uncertain reimbursement. For example, because of Prevnar 13's high cost,...

No see for 6 mos while on MAT? Only astronauts & military starting 6mo tour of Afghanistan.

Here's a better idea for the rest of our patients: 6 one-month prescriptions preceded by UDS & a small cash service fee...followed by a getting re-acquainted office visit.

I agree with Kevin, big time! I printed out the FINAL RULE (100+pages)..one section deferred to a later date. Worthwhile for those of us who take our bup program seriously. Isnt that everybody here?

Yes! One must be impressed with the amount of thought that went into the Final Rule. And I see they included just about every comment we questioners had, along with their responses.

Cheers,
Peter
