I lost a young girl after she violated parole, was incarcerated without any treatment of coarse. Upon release, she relapsed, overdosed and died. An all too common story.

I do agree that the Medical Board's primary objective is to serve and protect the public however, I find "compchat57" counterpoint a bit hard to swallow. I do not believe that a Medical Board would deny or suspend a medical license without due process.

My thoughts are that as with any opiate that is administered chronically, there will be a central reduction in LH and subsequently a reduced testicular production of testosterone. All chronically administered opiates will result in secondary hypogonadism.

My research has revealed that there is no benefit to a buprenorphine dose in excess of 24mg and in fact, present consensus is that the dose should rarely exceed 16mg. Near complete saturation of opioid receptors occur at the 16 mg dose.

Johns Hopkins 5 year data on their Buprenorphine clientele has shown that 50% of their patients are unable to extend their take-home dosing past 14 days without relapse. Therefore, I have only a rare patient that I will allow more than 14 days of take-home medications.

My practice has been to give an initial 1.4mg dose of Zubsolv followed by an additional 1.4mg in 1 to 2 hours. I have generally found that a second dose is usually required.

My understanding is that Vivitrol must be injected within one hour of being reconstituted. I agree with and appreciate the oral Naltrexone advice however, I would likely administer a 12.5 or 25mg dose.
